Webster Equity Partners, a private equity firm, has made an investment in Tri-Valley Medical Group, a physician platform that provides primary care and population health management services to senior and commercial members of southern California.

Tri-Valley has a 20-plus-year history in risk-based contracts with health plans and IPA partners. The company’s care model offers a suite of clinical services to patients and is used across outpatient clinics, nursing facilities and hospital settings. Ziegler, an investment bank, acted as financial advisor to Tri-Valley in this transaction.
