ClareMedica Health Partners, a provider of primary care services for Medicare Advantage patients and a portfolio company of Revelstoke Capital Partners, has acquired Plaza Medical Centers.
Plaza Medical Centers is a patient-focused primary care provider for Medicare Advantage patients. The company is focused on listening to patients and supporting them in their healthcare process. With this deal, ClareMedica aims to expand its care model in the Medicare Advantage sector.
“This acquisition nearly doubles the size of ClareMedica’s patient base to approximately ten thousand patients,” says Russell Cassella, managing partner at Revelstoke. “We believe the combined organization is well-poised for success under the leadership of Ren [Mullinix] and the ClareMedica team.”
