PointClickCare Technologies, a healthcare technology company backed by Hellman & Friedman and Dragoneer Investment Group, has bought Patient Pattern.

Patient Pattern is a value-based care electronic health record (EHR) and care management platform. The combination of PointClickCare and Patient Pattern aims to further the companies’ shared vision of working toward better care and outcomes for high-needs populations and providing career teams with services to successfully operate.

“We’re thrilled to join forces with PointClickCare as we march toward our collective goal of helping clinicians better predict and document risk, close care gaps, and improve patient outcomes,” says Steven Buslovich, co-founder and chief executive officer. “The marriage of an integrated care management platform with population health data will prove to be a major differentiator for us.”
