CoachCare, a remote patient monitoring and virtual health company, has acquired Alertive, a healthcare firm with a specialty in treating cardiology and nephrology patients.

CoachCare adds over 2,500 patients to its expanded chronic care management offering. Alertive’s services offering and clients are intended to complement CoachCare’s portfolio of primary care, obesity, pain management and orthopedic specialties.

“CoachCare continues to add specialties and services, such as chronic care management, to benefit more patients being treated across a number of primary and specialty conditions and within diverse insurance markets.
